Routine referrals
a) If your practice is signed up to Evolutio, refer using EOS web application
b) If your practice is not signed up Evolutio, refer using the GOS18 form (below) and then send via email (please ensure you send from a nhs.net email) to dbth.bookingsurgical@nhs.net
Bassetlaw
GOS18 referrals
All GOS 18 referrals for patients registered with a Bassetlaw CCG GP will be triaged via the Primary Eyecare Referral Assessment Service.
Send GOS18 Via nhs.net to cnech.pecservices@nhs.net
GP referrals (other health issues)
GOS 18 referrals for GP action should be directed to the GP and not via the referral assessment service.
