The new Wet AMD referral pathway went live as of 23rd of march 2020.

The form can be downloaded here “DBTH WET AMD FORM” filled out electronically and emailed.

Fill details, save and attach to the email address listed on the form.

The form can also be printed, scanned and email to the email address on the form.

Bassetlaw

Suspect WETAMD should be referred via Opera, using the CES option. OCT scans should be attached to the referral.
