The new Wet AMD referral pathway went live as of 23rd of march 2020.

The form can be downloaded here “DBTH WET AMD FORM” filled out electronically and emailed.

Visit https://www.dbth.nhs.uk/wet-amd-referral-form/ Downnload the editable pdf form by clicking on 'Click here' button

Fill details, save and attach to the email address listed on the form.

The form can also be printed, scanned and email to the email address on the form.

Bassetlaw

Suspect WETAMD should be referred via Opera, using the CES option. OCT scans should be attatched to the referral.

If non participating practice, send referral to Primary Eyecare Services Single Point of Access via cnech.pecservices@nhs.net
