Cataract Referals

Cataract referrals should be made via Evolutio

a) If your practice is signed up to Evolutio to provide Direct Cataract Referral, refer using EOS web application

b) If your practice is not signed up Evolutio, refer using the cataract referral form (below) and then send via email (please ensure you send from a nhs.net email) to dbth.bookingsurgical@nhs.net

Practices signed up to Pre/Post op cataract assessment service can manage the referral via Opera

Practices not signed up to Opera should refer via nhs.net to cnech.pecservices@nhs.net (please ensure you send from a nhs.net email). The patient will then be referred onto a participating practice.
